Migrant deaths in Mediterranean hit 'crisis proportions'

The UN refugee agency has raised the alarm after the feared drowning of 400 migrants in a shipwreck in the Mediterranean Sea, saying a “robust rescue-at-sea mechanism” is needed as more people take to the water to try to reach Europe.
