Misuse of rigging clips caused circus hair-hanging stunt to go wrong

The U.S. Occupational Safety and Health Administration says an improperly loaded caribiner clip is to blame for an aerial hair-hanging stunt accident that seriously injured eight acrobats in Rhode Island last May.
